Background: Sildenafil (Viagra®) is an oral drug that is used to treat erectile dysfunction (ED) and pulmonary arterial hypertension (PAH). The pharmacological action of sildenafil is mediated by the inhibition of phosphodiesterase type 5 (PDE5), an enzyme that produces cGMP (cyclic guanosine monophosphate), an inhibitor of cyclic guanosine monophosphate (cGMP). This article reviews the pharmacological properties of sildenafil and reviews how PDE5 was inhibited in patients with PAH.
Methods: A retrospective analysis was conducted of patients with PAH treated with sildenafil and those with erectile dysfunction (ED) in the United Kingdom. Patients were classified as responders (ED, ED >24) if the ED was not present on a daily basis. Sildenafil was used as a reference group for statistical analysis. Patients were further classified as non-responders (NRE) if the ED was not present on a daily basis. The primary outcome was the rate of erectile dysfunction (ED) as measured by a global assessment of erectile function (GFR).
Results: The average number of patients receiving sildenafil at baseline was 3.4, of whom 9 were responders. The mean duration of treatment with sildenafil was 4.1 months, with a median of 4.8 months (95% confidence interval [CI]: 3.3-5.0). The rate of ED as measured by the global assessment of erectile function (GFR) did not change significantly from baseline (6.3 vs 3.4, P=0.26).
Conclusion: Sildenafil is an effective drug treatment for ED in patients with PAH.
